NHS and a 2 tier health system

The creeping two tier health system was mentioned in the children for need thread and it struck a chord for a number of reasons, dont people who work in the private sector of health also work in the NHS? is the 2 tier thing then based solely on having a private hospital room to recover in, or is the need for private health care based on the failings on the NHS rather than a class thing or the ability of being able to afford it? Is private health care any better or is it just about jumping the queue? Does the quality of care and operations actually differ?
